If you are interested in becoming a New Patient of Mindshift Psychiatry,

Please follow the 2 easy steps below:

Choose an appointment date and time. An intake form will then be sent to your email or phone. The intake form has to be completed to verify eligibility. Please note that if using insurance, approval of schedule may be contingent upon eligibility and benefits.

After Booking your Appointment online, please check your spam messages in email for the Intake Form. This form is needed for eligibility checking. You will receive a confirmation via email or text once your appointment request is approved. All forms are electronic and must be completed prior to your appointment time.
